The Role of a Family Worker by Meena Mehta
My role is to support and help inform transition planning and provision for Students from Yr 9 – Yr 14 at the Village School and liaise with Local Education Authorities to look at appropriate further Education placement.
To organise and facilitate The Annual Village School Transition Event during the autumn term primarily for our leavers but also for students in Yr9 to YR14 and their parents and carers. It enables them to meet with local statutory and independent providers and professionals and look at the services they can offer them.
To liaise and work with all professionals/services involved with the young person/leavers to ensure that the transition process is smooth and has the best outcome for the young person.
Support Young People and involve young people in the process of Person Centred Planning, this allows for the young person’s views and aspirations to be heard and become the focus of meetings with professionals to help formalise a plan for them when they leave the Village School.
Support parent and carers and involve them in the process and help them to understand the different pathways and put forward their views and aspirations for their children towards the Person Centred Planning and other meetings including Annual reviews and EHCP plans.
Support with College visits and empower them to attend College Open days.
Support them to fill in and understand relevant paperwork involving the transition process; transport request forms, Education, Health & Care Plans, etc.
Sign post parents and young people to Statutory (Social Care, Colleges, Brent Respite, Therapy Services, Connexions) and Non statutory services including national( NAS, SCOPE, and local support groups/charities (Brent PCF, Brent Somali Community Centre, Brent Carers Centre, Brent Mencap, Brent Citizens Advice Bureau.
